Physical Therapist

Bridgeport Connecticut Seeking a Physical Therapist. The candidate provides and directs Physical Therapy services to residents to include assessment and treatment, development and implementation of programs, and related professional in-services. Performs duties under the direction of physician's orders. Responsibilities: Provides Physical Therapy services to residents and initiates referrals when appropriate and in accordance with Physical Therapy Department procedures. Conduct screenings of residents at regular intervals to determine need for intervention/treatment. Evaluates residents to obtain data necessary for treatment planning and implementation. Conducts specialized evaluations as indicated. Interprets and communicates evaluation findings to residents, family, and other team members. Monitors resident's response to intervention and modifies treatment as indicated to attain goals in accordance with physician's orders. Participates in discharge planning including programming to maintain and enhance the performance of the resident in his or her own environment. Documents results of resident's assessment, treatment, follow-up, and termination of services.
Reviews the quality and appropriateness of the total services delivered and of individual Physical Therapy programs for effectiveness and efficiency, using predetermined criteria. Supervises Rehabilitation Aides, students, volunteers, directs LPTAs in the implementation of the treatment plan. Assumes responsibility for ongoing continuing education and professional development. Complies with all Infection Control, Universal Precautions and OSHA standards for the health care professional. Assists with and participates in Quality Assurance Programs as required. Bachelor of Science in Physical Therapy from an accredited program required.
Successful completion of National Certification Examination for Registered Physical Therapist required. Current state licensure or licensure eligible.
